It's insurance renewal time and I thought it was best to request each of you to answer the statements below and modify the flight times as appropclate, I have filled ln the columns with the best data I know. Please change the data if it is not accurate.

PLEASE SIGN AND RETURN THIS COMPLETED PAGE TO ME AS SOON AS POSSIBLE. I will transfer the info to the insurance renewal form.

ANSWER THE FOLLOWING QUESTIONS (Yes or No, if you answer any Yes, please give details under remarks):

1. Had any accidents or citations for F.A.R. violations?
2. Had any license limitations or waivers?
3. Had any felony convictions or license suspensions arising out of operation of a motor vehicle?
4. Had any arrests for operation of motor vehicle recklessly or under influence of alcohol or drugs?
